Stomach sleepers need the thinnest, softest pillow they can tolerate. Here is the right loft, the fills that work, and the pillow that matters more than the one under your head.

If you sleep on your stomach, almost every pillow sold is wrong for you. The short answer is that you want the thinnest, softest pillow you can find, somewhere around two to three inches of loft, and in some cases no pillow under your head at all.
That sounds like a strange thing for a bedding store to say, so here is the reasoning.
Lying face down, your head has to turn roughly 90 degrees to one side so you can breathe. Your neck is already at the far end of its rotational range before a pillow is involved.
Now add height. A standard pillow, built for a side sleeper who needs to fill the gap between shoulder and ear, lifts your head three to five inches off the surface. On your stomach, that lift pushes your head backward into extension while it is already rotated. You are asking your cervical spine to hold two demanding positions at once for seven hours.
The usual result is waking with a stiff neck, a sore shoulder on the side you face, or a dull ache at the base of the skull. Most people blame the mattress. It is usually the pillow.
Loft is simply how tall the pillow sits when your head is on it, which is not the same as how tall it looks in the packet. A plush pillow that measures five inches on a shelf may compress to two under head weight, and that is fine. A firm one that measures four will stay close to four, and that is not.
For stomach sleeping, the target is a compressed height of roughly two to three inches. The test is simple: lie face down in your normal position and have someone look at you from the side. Your head, neck and upper spine should form close to a straight line. If your chin is tipped up and back, the pillow is too tall.
A reasonable number of stomach sleepers genuinely do best with nothing under the head at all. If that is comfortable for you, it is not a problem to be corrected.
Down and down alternative. The most forgiving option. Both compress a long way under the weight of a head, and you can usually redistribute the fill by hand to flatten the middle. Look for a low fill weight rather than a luxurious one, since a densely stuffed down pillow behaves like a firm pillow.
Shredded memory foam with a removable fill. The useful feature here is not the foam, it is the zipper. Pillows sold with an adjustable fill let you take out handfuls until the height is right. For a stomach sleeper, that adjustability matters more than anything else on the spec sheet, because almost every pillow ships with too much in it.
Thin fiber or microfiber pillows. Cheap, widely available and easy to replace. They flatten quickly, which is a drawback for most sleepers and a feature for this one. The trade-off is that they flatten unevenly and need replacing more often.
Low-profile latex. Latex is more durable than foam and more breathable, which is worth something in a warm Los Angeles bedroom. Only buy it in a specifically low-profile version, and be aware that latex is springy and will not compress much once you are on it. What you see is close to what you get.
Here is the piece most guides leave out. Put a thin pillow under your pelvis and lower abdomen.
Stomach sleeping lets the middle of your body sink into the mattress, which arches the lower back. That sustained arch is where a lot of stomach sleepers' morning back stiffness actually comes from, and no head pillow addresses it. A flat pillow or folded blanket under the hips lifts the pelvis just enough to reduce the arch and flatten the lumbar curve toward neutral.

The arch problem above is created by the mattress, not the pillow. A soft mattress lets the hips drop further than the shoulders, and a hip pillow is a patch over that.
Stomach sleepers generally do better on a firmer surface, in the medium-firm to firm range, which keeps the pelvis level with the rib cage and the shoulders. If your neck pain is persistent, does not settle during the day, or comes with numbness or tingling in an arm or hand, that is worth raising with a doctor rather than solving with bedding.
Because stomach sleepers rely on a pillow staying thin and even, a pillow that has gone lumpy is a bigger problem for you than for a side sleeper who can just fluff it. The thinnest one you find comfortable, ideally compressing to around two to three inches under the weight of your head. Soft down, down alternative, or a shredded fill pillow you can remove stuffing from are the three most reliable options, because all three let you get the height down. Firmness is not a virtue here.
Adjustable-fill pillows are the most practical type, because almost every pillow on the market ships with more fill than a stomach sleeper wants. Being able to unzip it and remove handfuls until your head sits level with your spine beats trying to find a pillow that happens to be thin enough out of the box.
Yes, and it helps more than most people expect. Place a thin pillow or folded blanket under the pelvis and lower abdomen. It lifts the hips slightly and reduces the arch in the lower back that stomach sleeping creates. If you only change one thing after reading this, make it this one.
Not at all. For many stomach sleepers it is the better option, because it keeps the neck closest to neutral. The only reasons to use one are comfort and a bit of cushioning under the cheek. If going without feels fine, there is no anatomical reason to add one back.
It is the position that puts the most sustained rotation through the neck and the most arch through the lower back, so it is generally considered the least kind to the spine. That said, the position you actually fall asleep in tends to win, and forcing a change often costs you more sleep than it saves. A thin head pillow, a pillow under the hips and a firmer mattress address most of the downside without fighting your habits.
Almost always because the pillow is too tall. Your head is already rotated close to 90 degrees to breathe, and a pillow built for a side sleeper adds backward extension on top of that rotation. Holding two end-range positions at once for several hours is what produces the morning stiffness. Lower the pillow and it usually resolves within a week.
Pillow height is almost impossible to judge standing in an aisle. The only useful test is getting into your actual sleeping position and having someone check whether your head, neck and spine line up.
