Bed bugs do not travel on their own. They arrive in luggage, and the most common way a household gets them is a suitcase that came home from a hotel. A five-minute inspection before you unpack is the difference between a good trip and a problem that costs you a mattress.
Here is the inspection, in the order a pest control technician would do it.
Before you check anything, put your bags in the bathroom. Tile and porcelain give bed bugs nowhere to hide, and the bathroom is the one room in a hotel they almost never occupy. Do not set a suitcase on the bed, the carpet, or an upholstered chair while you inspect. If the room turns out to be infested, that one habit is what keeps the bugs out of your bag.
You will rarely see a live bed bug during the day. What gives them away is the evidence they leave behind:

Pull the sheets back from the head of the bed and expose the bare mattress. Run your light along the piping on all four edges, paying particular attention to the top third of the mattress, nearest where a sleeper's head and shoulders would be. Bed bugs stay close to where they feed.
Check the tag and any label seams; the folded fabric is a favorite harborage. Then lift the mattress at one corner and look at the underside edge and at the top surface of the box spring beneath it.
The box spring is where infestations usually concentrate, because the fabric is loose and the wooden frame inside gives them crevices. Look along the stapled gauze underneath if you can see it, and check the corners of the frame.
Then check the frame itself: screw holes, joints, the point where the headboard meets the wall. Wooden frames hide more than metal ones.
Hotel headboards are usually mounted on wall brackets and lift off. If yours does, lift it slightly and look at the back and at the wall behind it. This is the single most productive spot in a hotel room, and it is the one most guests never check. If the headboard is fixed, run your light along its top edge and into the gap at the wall.
Pull out the nightstand drawer entirely and check the inside of the cavity, the drawer runners, and the underside. Then inspect the luggage rack, especially the webbing straps and the joints where the legs fold. A luggage rack carries bugs directly to the next guest's suitcase, which makes it worth thirty seconds of attention.
Check the seams and skirt of any armchair or sofa, and the folds at the bottom of floor-length curtains. These are secondary sites, so if the bed is clean they usually are too, but a heavy infestation will have spread.
Do not move to another room on the same floor. Bed bugs spread laterally through wall voids and shared plumbing, so adjacent and directly adjoining rooms carry the highest risk. Ask for a room several floors away, or change hotels. Report what you found: a reputable property will want to know, and a property that reacts badly has told you something useful.
If your luggage was ever on the bed or the floor of that room, treat it as exposed and follow the precautions below when you get home.
This part matters more than the hotel inspection, because it is your last line of defense. Unpack in the garage, on a patio, or in a bathroom, never in the bedroom. Put everything washable, worn or not, straight into the dryer on high heat for thirty minutes. Heat kills bed bugs at every life stage, including eggs, and the dryer is more reliable than washing. Vacuum the empty suitcase inside and out, then empty the vacuum outside the house immediately. Store the suitcase somewhere other than the bedroom closet.

Bed bugs are not a hygiene problem and they are not a reflection on you. They are a travel problem, and the fix is an inspection habit rather than a cleaning habit.
Put your luggage in the bathroom first, then pull back the sheets and inspect the mattress seams and piping with a flashlight, focusing on the top third. Lift the mattress corner to check the box spring, then lift the headboard off the wall. Look for dark rust-colored specks, pale shed shells, and clusters of tiny white eggs rather than for live insects.
The reliable signs are physical evidence in the bed, not the room's general appearance. A spotless, expensive hotel can have bed bugs and a basic one can be clean, because infestations come from guests rather than from housekeeping. Dark specks along mattress piping and shed casings in the box spring seams are what tell you.
In descending order of likelihood: behind the headboard, in the box spring seams and interior, along the mattress piping and tag, in the bed frame joints and screw holes, inside the nightstand drawer cavity, and in the luggage rack webbing. They stay within a few feet of the bed because that is where they feed.
Leave the room with your luggage, tell the front desk what you found and where, and ask to be moved several floors away or request a refund and book elsewhere. Avoid an adjacent or directly adjoining room. When you get home, run everything washable through a hot dryer for thirty minutes before anything enters the bedroom.
