Cooling pillows use gel, phase change fabric, breathable fills and airflow channels to pull heat away from your head. Here is what actually works and how to choose one.

Your head gives off a surprising amount of heat, and a pillow sits right against it for eight hours with very little airflow. That is why the pillow is often the first thing that feels too warm at night, and why flipping to the cool side is such a universal habit.
A cooling pillow is built to delay that warm-up. Some do it well. Some rely on marketing language that means very little. Here is how to tell the difference.
Two things: material density and trapped moisture.
Solid memory foam is the usual culprit. It is dense, air does not move through it easily, and it softens in response to body heat, which reduces the small air gaps that would otherwise carry warmth away. Add a tightly woven polyester cover and heat has nowhere to go.
Moisture makes it worse. Even light perspiration gets absorbed by the pillow and cover, and damp fabric feels warmer and clammier than dry fabric at the same temperature. A pillow that manages moisture often feels cooler than one that only manages heat.
Gel beads mixed into foam, or a solid gel pad laminated to one side, conduct heat away from your skin faster than foam alone. The effect is real but finite. Gel pulls heat until it reaches your body temperature, then it stops helping. Expect a noticeably cooler first stretch of the night rather than all-night cooling.
This is one of the most effective and least advertised features. Shredded memory foam has air gaps throughout, so heat and moisture escape instead of building up. Shredded fill is also adjustable, since you can remove or add material to change the loft.
PCM fabric absorbs heat as it changes state, then releases it as it cools. Covers made with PCM feel genuinely cool to the touch and, unlike a static gel pad, can recharge through the night. It is one of the more effective options and usually costs more.
Holes punched through the foam create airflow channels. Simple, inexpensive, and it works, particularly when combined with a breathable cover.
Natural latex has an open-cell structure and a pinhole core, so it breathes far better than solid memory foam while still offering support. It is naturally resistant to dust mites, which appeals to allergy-prone sleepers.
An old solution that still works. The hulls do not retain heat and air moves freely between them. The trade-offs are weight, firmness and a rustling sound when you move.
Easy to overlook and genuinely important. Cotton percale, bamboo-derived viscose, Tencel and PCM knits all breathe and wick better than standard polyester. A great cooling core inside a sealed polyester cover will still sleep warm.
A cool pillow that puts your neck at a bad angle is still the wrong pillow. Get the loft right first, then optimize for temperature.
Side sleepers need the highest loft, because the pillow has to fill the gap between the ear and the outer shoulder. Back sleepers need medium loft to support the natural curve of the neck without pushing the head forward. Stomach sleepers need the thinnest pillow available, or none at all, to avoid craning the neck upward.

LA bedrooms run warm for a long stretch of the year, and a lot of the housing stock here was not built with overnight cooling in mind. Older apartments and second-floor units hold the day's heat well past midnight, and plenty of buildings have no central air at all.
If that describes your place, treat the pillow as one part of a system. A cooling pillow on a mattress that traps heat only solves part of the problem. Pairing it with a breathable mattress topper or a cooling-focused mattress makes a much bigger difference than any single item. Start with your sleeping position and pick the right loft. Then choose a construction that breathes: shredded fill, latex, or a ventilated core rather than a solid foam block. Prioritize the cover, since a PCM or natural-fiber cover does a lot of the work. Treat gel as a helpful bonus rather than the main feature, and set expectations accordingly.
If you run hot and have the budget, a PCM cover over shredded latex or shredded foam is the combination that tends to satisfy people longest.
They use some combination of four mechanisms: conduction, where gel or PCM draws heat away from your skin; airflow, through shredded fill, perforations or open-cell latex; moisture wicking, via breathable cover fabrics; and reduced density, so less material means less heat retention. Most good cooling pillows combine two or three of these rather than relying on one.
Yes, though the effect varies by design. A pillow with shredded breathable fill and a PCM or natural-fiber cover will noticeably outperform a solid memory foam pillow in a warm room. A gel pad alone gives a cool initial feel that fades once it equalizes with your body temperature. Manage expectations: a cooling pillow delays warming, it does not refrigerate.
Check the care label first, because construction varies. Most removable covers are machine washable cold on a gentle cycle. Solid foam and gel-layer cores should never go in a washing machine, since agitation breaks down the foam. Spot clean those with mild detergent and let them air dry completely. Gel pillows sold by established manufacturers are safe for normal use. Look for CertiPUR-US certification on foam components, which verifies the foam is made without certain regulated substances and meets low VOC emission standards. New foam pillows may have a mild off-gassing odor for a few days, which fades with airing out.
The bamboo-derived viscose used in pillow covers is breathable and wicks moisture well, so it does feel cooler against the skin than standard polyester. Keep in mind that most bamboo pillows are shredded memory foam inside a bamboo-blend cover, so the cooling comes from the cover and the shredded fill working together rather than from bamboo itself.
