How many pillows a bed needs and where each one goes, with layouts by mattress size and a simple rule for keeping the arrangement practical rather than decorative clutter.

A well-arranged bed follows one structural idea: pillows go in rows, back to front, largest and firmest at the back, smallest and softest at the front. Everything else is a variation on that. Once you see the rows, the arrangements that look expensive in hotel photographs stop looking like a mystery.
The other thing worth knowing up front is that only some of these pillows are for sleeping on. Mixing the two jobs is what makes a bed look cluttered and makes the nightly reset annoying.
Against the headboard sit your largest pillows, standing upright. These are usually euro squares, which are square pillows most often 26 by 26 inches. They do the structural work: they fill the gap between the mattress and the headboard, and they give the rows in front of them something to lean against. Sleeping pillows can play this role instead if you would rather not buy euros.
Your actual sleeping pillows, in pillowcases or shams, leaning slightly against the back row. This is the row you use every night, so it should be the one that stays easiest to reach.
One or two decorative pillows, usually a lumbar (a long rectangle) or a smaller square. This row is optional and its only job is to add a colour or texture note. Keep it to one or two, because everything in this row has to come off the bed every night.

King pillows, not queen, are what the front row wants here.
Keep the arrangement light. Decorative pillows have to be moved every time the base articulates, and euros wedged against a headboard get compressed when the head section rises. Two sleeping pillows per side and one accent pillow for the whole bed is the practical limit on an adjustable bed.
Even numbers in the structural rows, odd at the front. Two euros and one lumbar looks composed. Two euros and two lumbars looks like a display.
A bed with five pillows of three different sizes reads as layered. Five pillows of the same size in different fabrics reads as a pile. Size contrast is doing the work.
Euros upright and flat against the headboard. Sleeping pillows tipped forward a few degrees. Accent pillow propped against the sleeping pillows. That slight forward cascade is why hotel beds look deliberate.
The dented top edge on a decorative pillow is a styling convention, not a rule. It suits down and down-alternative inserts and looks wrong on a firm foam insert. Do it to all of them or none of them.
If you will not move six pillows every night, do not put six on the bed. A three-pillow arrangement you actually maintain looks better than a six-pillow arrangement that ends up on the floor.
None of the above says anything about whether your pillows are any good to sleep on. A decorative arrangement can sit on top of pillows that are wrecking your neck. Those are different purchases with different criteria: loft and fill for the sleeping pillows, size and cover fabric for the rest. Five works for most people: two euro squares at the back, two queen sleeping pillows in front, one lumbar accent pillow. Three is a perfectly good minimum if you want the bed simple, and going above six on a queen starts to look crowded.
Six, in three sizes: three euro squares across the back, two king sleeping pillows, one long lumbar. A king is wide enough that two euros leave a visible gap, which is why three is the standard for that size.
Put them in the front row only, keep them to one or two, and make them a different size from everything behind them. A lumbar rectangle in front of square and standard pillows gives the contrast that makes the whole arrangement read as layered rather than repetitive.
A euro pillow is square, most commonly 26 by 26 inches, and is meant to stand upright as a back support or a structural layer. A standard pillow is rectangular and meant for sleeping. Three things help. Stand the back row fully upright so it braces against the headboard rather than the mattress. Make sure the mattress sits close to the headboard with no gap. And use firmer inserts in the back row, since soft euros collapse forward and take the rows in front of them with them.
The accent and euro rows come off. A basket, bench or shelf at the foot of the bed is the usual answer, and having a designated spot is what makes people keep doing it. If there is nowhere obvious to put them, reduce the count instead.
