Modern certified electric blankets are safe for most adults, but age, damage and who is under them change the answer. The real risks, who should ask a doctor first, and a nine-point safety check.

Short answer: a modern, certified electric blanket in good condition, used the way the manufacturer intends, is safe for most healthy adults. Almost every serious incident involves one of three things instead: a blanket more than a decade old, physical damage to the internal wiring, or a user who should not have been under one in the first place. All three are checkable, which makes this one of the rare safety questions with a practical answer.
An electric blanket is a quilted or fleece blanket with thin insulated heating wires threaded through the fabric in a looping pattern. A controller regulates how much current reaches those wires, which is how you get heat settings. Most blankets sold today also include a thermostat that cuts power to any section that gets too hot, plus an automatic shut-off timer.
Two relatives get lumped in. A heated mattress pad sits under the fitted sheet rather than over you, warming the bed more evenly. A heated throw is a sofa-sized version usually not rated for sleeping under all night. The advice below applies to all three.
This is the big one. Heating wires fatigue where the blanket folds, creases or gets crushed between a mattress and a wall, and a broken wire can arc and scorch the fabric around it. Consumer safety guidance generally points to replacing an electric blanket roughly every ten years, sooner if it has been washed many times or stored folded tightly.
Each season, lay it flat under good light and look for scorch marks or discolored patches, frayed or cracked cord insulation, a loose controller connection, and lumps or hard kinks where a wire may have snapped. Any of those means the bin, not the bed.
Electric blankets do not get hot enough to burn you quickly, but over hours they can cause low-temperature burns on skin that stays in one spot, particularly for people who cannot feel the heat properly or move away from it. Hence the safest routine: preheat the bed for twenty or thirty minutes, then switch the blanket off when you get in.
A blanket that is folded, bunched or buried under a thick duvet concentrates heat in one area. Never run one folded, never let a pet sleep on top of it, and do not wedge the cord or controller under a mattress where it can be pinched. Electric blankets also do not belong on adjustable bases or folding beds unless the maker approves it, because repeated bending stresses the wiring at the hinge points, and nothing electric belongs on an air mattress.

The safest routine is to preheat the bed and switch it off before sleeping. People with reduced skin sensation, circulation problems or limited mobility, and anyone pregnant, should check with a doctor first, and electric blankets should not be used for infants or young children.
Many modern blankets include a timer and a thermostat so they can run longer, and sleeping under one on a low setting is common. It is still not the lowest-risk option, because prolonged contact heat is what causes low-temperature burns. If preheating and switching off gets you the same comfort, do that.
Research into low-frequency electromagnetic fields and electric blanket use has not established a causal link, and modern wiring designs substantially reduce the field in the first place. If the question still bothers you, turning the blanket off before you fall asleep removes most of the exposure without giving up the warm bed.
