No, you do not need a headboard. Nothing about a mattress, a foundation or a bed frame requires one, and plenty of people sleep perfectly well without one for years. But that is the boring answer, and it is not really what people are asking. The real question is whether a headboard earns its cost and its floor space in your bedroom, and that depends on how you use your bed.
It protects the wall. This is the one people underestimate. Without a headboard, your pillows, your hair oils and the top edge of the mattress spend every night in contact with painted drywall or plaster. Over a couple of years that leaves a grey smudge that does not wash off, and in a rental that is a deposit conversation.

One Los Angeles-specific point worth making: if you wall-mount a heavy headboard, anchor it into studs, not drywall anchors alone. A headboard that comes off the wall during a quake lands exactly where your head is. It takes ten extra minutes with a stud finder and it is not optional.
Upholstered is the most comfortable to lean on and the most forgiving in a small room, because fabric reads softer than a big wooden slab. The trade-off is cleaning: hair products and skin oils show on light fabric, and you will be spot-cleaning it. Performance fabrics and darker colours help.
Wood is durable and easy to wipe down, but hard against your back and prone to knocking against the wall if the frame shifts. Felt pads on the back solve the noise.

No. No mattress, foundation or warranty requires one. They solve real problems, particularly wall staining and sitting up in bed, but you can address both with a wall-mounted panel, washable paint or a good pile of pillows if you would rather not buy one.
Not quite. Most standard metal frames and foundations have pre-drilled bracket holes, and most headboards are built for those, but hole spacing and height vary. Platform beds and adjustable bases frequently have no mounting points at all, in which case you either use a compatible bracket kit sold for that base or mount the headboard to the wall instead.

No, and many platform beds are designed to look complete without one. If you want the wall protection and back support anyway, a wall-mounted or freestanding headboard is usually easier than trying to bolt one to a platform frame that was never drilled for it.
