Mold on a mattress can sometimes be cleaned and sometimes cannot. Here is how to tell which situation you are in, how to remove surface mold safely, and the point at which replacing the mattress is the right call.

Mold on a mattress usually announces itself before you see it. There is a musty, basement-like smell that does not wash out of the sheets, and when you finally strip the bed you find grey, black or greenish speckling, most often on the underside or along one edge. The uncomfortable truth is that mold can sometimes be removed and sometimes cannot, and the difference comes down to how deep it has gone. Not every dark stain is mold. Old sweat and body oil oxidize into yellow-brown patches that look alarming but are not fungal. Mold has three giveaways:
Flip the mattress on its side and look at the bottom panel and the perimeter. Mattresses that sit on a solid platform, on a slatted base with wide gaps that trap condensation, or directly on the floor almost always show it on the underside first.
Work in a well ventilated room with a window open, or better, take the mattress outside. Wear gloves and an N95 mask. Disturbing mold releases spores, and the point of this job is to remove them, not redistribute them through your bedroom.
Set your expectations honestly. You can treat mold on the surface fabric and the first fraction of an inch beneath it. You cannot reach mold that has grown into foam layers or into a coil unit, because you cannot get cleaning solution in there and, more importantly, you cannot get the moisture back out. Any method that soaks a mattress to kill mold creates the exact conditions that caused the mold.
Use a vacuum with a HEPA filter and an upholstery or crevice attachment. Go slowly over the mold and about a foot around it, then over the seams and the opposite face. Take the vacuum outside immediately afterward, empty the canister into a sealed bag, and wash the attachment. Skipping this step means you spread spores around with every later step.
Mix equal parts 70 percent isopropyl alcohol and warm water, or use undiluted white vinegar. Dampen a cloth, do not saturate it, and work the solution into the moldy area in small circles. Alcohol evaporates faster than vinegar, which is an advantage on foam. Vinegar is more effective on fabric but takes longer to dry.
Resist the urge to pour anything on. The single biggest mistake people make is using so much liquid that the mattress stays damp for days, which guarantees the mold returns worse than before.
Go over the area with a cloth dampened in plain water to lift residue, then press a dry towel firmly into the spot to pull moisture back out. Repeat with fresh dry towels until almost nothing transfers.
This is the step that decides whether the job worked. Stand the mattress on its side so air reaches both faces. Run a fan directly at it, and a dehumidifier in the room if you have one. If you can get it into direct sunlight for a couple of hours, do, since UV light helps kill residual spores, though do not leave foam baking in the sun all day.
Give it a minimum of eight to twelve hours before putting bedding back on, and longer if the room is humid. The mattress should feel completely dry to the back of your hand, not merely cool.

Mold is a symptom. If the mattress was on the floor, get it onto a slatted base. If the room runs humid, keep it under 50 percent with a dehumidifier or air conditioning. If a spill started it, use a breathable waterproof protector going forward. See whether you can put a mattress on the floor and whether you need a mattress protector.
Bleach is a poor choice on a mattress. It discolors fabric, degrades foam and fibers, and on porous surfaces it tends to kill the surface growth while leaving the roots. It also leaves a residue you then sleep on.
Steam cleaners and carpet shampooers are worse. Both inject significant moisture into a material that cannot be wrung out or air dried quickly, which is how a small mold patch becomes a whole-mattress problem.
Essential oils, fabric refreshers and odor sprays mask the smell without touching the mold. If the musty smell comes back a week later, nothing was actually removed.

Vacuum the area with a HEPA vacuum, treat it with a cloth dampened in diluted rubbing alcohol or white vinegar rather than soaking it, blot thoroughly dry, then stand the mattress on edge with a fan on it for at least eight to twelve hours. Finish with baking soda to absorb residual odor. This works on surface mold only.
Clustered grey, black, green or white speckles with a slightly fuzzy, raised texture, most often on the underside or along one edge. Flat yellow-brown patches are usually oxidized sweat and body oil, not mold. The reliable tell is the smell: mold produces a persistent musty, earthy odor that intensifies when you press on the mattress.
Sleeping directly above mold means breathing spores for seven or eight hours a night at close range. For most healthy adults the result is irritation: congestion, a cough, itchy eyes, disrupted sleep. For anyone with asthma, allergies or a weakened immune system it can be considerably more of a problem. If symptoms improve when you sleep elsewhere, take that seriously, and speak to a doctor about any persistent respiratory symptoms.
Strip it completely, stand it on its side, and inspect both faces and the perimeter in good light. Check the underside first, since that is where airflow is worst and mold usually starts. A musty smell with no visible staining still points to mold, often inside the mattress or on the base beneath it, so check the slats and platform too.
Surface mold on the cover, yes, with the alcohol method and thorough drying. Mold that has penetrated the foam itself, no. Memory foam is dense and closed enough that you cannot flush a cleaning solution through it or dry the interior afterward, so anything beyond surface treatment means replacement.
Yes, and it is one of the most common causes. Body heat and perspiration push moisture downward through the mattress overnight, and with no air gap beneath it the moisture condenses against the floor and has nowhere to go. A slatted base or a platform with ventilation gaps solves it.
